Sick (fit) notes
If you’ve been off sick for more than 7 days
To request a sick note:
- fill out a sick note request form
- phone or visit the surgery from 10am to 6pm
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Hospital Referrals
Should you be referred to the hospital we will ensure that your referral is processed within four working days or arranged on the same day if the doctor feels it is very urgent.
Decisions on your treatment, including the options open to you such as choice of hospital, will be discussed fully with you.
Please inform your doctor if you wish to be referred privately.
If you have any queries regarding your referral after you have seen the doctor, please contact the surgery and ask to speak to your doctor’s medical secretary.
Non-NHS Services
Certain examinations are not covered by the NHS.
These include medicals for HGV licences, PSV licences, fitness for occupation, travel and sporting activities etc, together with some medical certificates.
All of these services can be arranged via the reception staff who will inform you of the fee involved.
Fees should be paid in advance of collecting forms and certificates.
Minor Injury
Did you know...
You do not need to go to A&E if you have got a minor injury. It could be far quicker and more convenient to see us at the surgery. Ring us and we will fit you in on the same day.
By minor injuries, we mean conditions like:
- cuts and grazes
- neck pain (unless following a bang on the head or high-speed crash)
- wound infections
- insect and animal bites
- minor bumps to the head
- injuries to the back, shoulders or chest
- minor burns or scalds
Test Results
As a practice we do not contact you with your blood results if they are normal.
If your results are abnormal or the doctor wishes to discuss your results with you, the surgery will contact you.
You can telephone through for your results, should you wish, but please leave at least two weeks to allow for the results to be processed at the Path lab and seen by the doctor. If you do telephone the surgery for your result please ring after 11.00am. Alternatively, it may be easier to register for online access to your record which includes being able to see your test results.
Please be aware, as with all patient information, we cannot disclose your test results to anyone other than you unless specifically authorised by you.
Once a doctor has reviewed your test results, you can view them:
- in your NHS account (through the NHS website or NHS App)
Stop Smoking
Local stop smoking services are free, friendly and can massively boost your chances of quitting for good.
These services staffed by expert advisers provide a range of proven methods to help you quit.
They’ll give you accurate information and advice, as well as professional support, during the first few months you stop smoking.
They also make it easy and affordable for you to get stop smoking treatments, such as nicotine replacement therapy or bupropion (Zyban).

Emergency Contraception (Morning After Pill)
Emergency contraception can generally be provided up to five days after unprotected intercourse.
Please make an emergency appointment with either a doctor or nurse if you think you might need emergency contraception.
Remember, the sooner you use emergency contraception the more effective it is, so don’t delay!
