Why The Fountain?
We previously
worked in a converted house on London Road, Newark, and moved here in October
1986. The old surgery was by a small public garden known as the Fountain Gardens.
The Fountain was a monument erected at the beginning of the century to Ethel
Harrison who rescued a child from the Chesterfield canal but perished in the
attempt. Many of our older patients referred to our premises as the "Fountain" and
we have tried to preserve the link by incorporating a small fountain in the design.
Our Surgery

Practice News and Notices
The surgery will be closed for training on Wednesday 18th March from 12pm onwards. Contact details for the emergency doctor are: 0115 934 3028
If you are travelling abroad to one destination we can offer basic travel advice, please ask for a Travel Form from reception. You will need to complete this form and return it to the surgery for one of the nursing team to address. We advise that you do this at least 8 weeks before you travel. Please note that any prescription ordered for collection by you will be ready within 48 hours. If you have a chemist collect your prescription you may need to allow them an additional day, please check with your pharmacist. Please check with our Prescriptions department for details on Chemist collections. You can email your prescription to the surgery fountain.prescription@gp-c84019.nhs.uk
Please let us know if you are a Carer for a patient. We have a form for completion available from the Reception staff.
